Sorry I'm late to the party.  I think you've figured out a solution but
I'll chime in anyway.

Based on the solution, I suspect that part of the problem is a
misunderstanding of the collapse_features option.  When on, it only
hides features that have the exact same image drawn on the exact same
spot (ie two marker that are so close together that their triangles
would be drawn at the same spot).

What you want is the feature_default_display.  Off the top of my head
(and Dave will have to confirm this), I think all you need to add is a
ft_DEFAULT=2 to the url, which will set all the feature types to always
display.  

You can also specify the feature_default_display value (display,
corr_only, ignore) in the configuration for each feature type.
